A completed booking form is required to record details relevant to the production of a wedding video and is available on request just e-mail or telephone and a booking form will be sent to you by return. The booking form will prompt you to consider the content and options you may want to include in your wedding video production such as Names for Titles/Credits, Childhood Photographs, DVD format, Consent to video the marriage ceremony, Music selection and your contact/address information.
